Forms & Policies

Please fill out the New Patient Form prior to your first appointment.

We request co-payments be made at registration. Due to the constant change in insurance policies and coverage, we recommend checking your coverage with your insurance provider. Any balance unpaid by the insurance company will be deemed patient's responsibility and charged accordingly.

Patients who do not show up to their appointment and who do not provide the medical center with 24 hours notice of their appointment cancellation will charged a No Show Fee.

Patients who arrive late to their appointment may be asked to wait until an opening is available. We may request you to reschedule your appointment if no opening is available. We try our best to keep to the schedule, so we request our patient's arrive promptly for their appoints as well.
